The Individual acknowledges receipt of the animal described below and that by taking possession of the animal this transfer is final.
The Individual understands that the Humane Society for Hamilton County (HSHC) makes no representation or guarantees about the animal’s health, temperament, breed or mental disposition.
The Individual accepts this animal as is and assumes all risks and responsibilities of ownership of the animal including but not limited to, any injury or damage caused by the animal (e.g. animal bites) and any treatable medical conditions that should arise.

HSHC Terms and Conditions of Pet Ownership. Please check to indicate that you have read the following:*

The Individual will ensure this animal is placed in a home where it will be provided with proper food, clean water, suitable living conditions, and all necessary medical treatment. Suitable living conditions include but are not limited to: Shelter from the elements which keeps your pet cool in the summer, warm in the winter, and dry. Animals do get frost bite, so it is expected that special accommodations will be made to keep your pet safe and healthy during extreme weather conditions.
The Individual agrees that this animal will not be used for breeding, experimental, inhumane or illegal uses (e.g. dog fighting) or placed with an adopter or foster home where any of these activities are possible.
The Individual agrees that if for any reason they can no longer keep this animal or deem it necessary to euthanize this animal, the HSHC will be n otified immediately and provided the opportunity to take the animal back into our care.
The Individual agrees to surrender ownership of this animal to the HSHC if the Humane Society determines that the animal’s living conditions are not satisfactory, or if the Organization is found in breach of any portion of this adoption contract.
The individual agrees to care for cats in accordance with Hamilton County’s Feral Cat Ordinance
